ABI Long-term Care Waiver

The Acquired Brain Injury (ABI)Long-Term Care waiver is an alternative to institutional care for individuals who have reached a plateau in their rehabilitation level and require maintenance services to avoid institutionalization and to live safely in the community.

Hart-Supported Living Grant

The Hart-Supported Living program is for any eligible Kentuckian with a disability recognized under the Americans with Disabilities Act (ADA).

HCB Waiver

The Home and Community-Based Waiver (HCB) provides eligible persons with Medicaid coverage who are aged or disabled, who would otherwise require nursing facility level of care.
